We are a collective movement to support people with long term health conditions including heart disease, stroke, MS, dementia, Parkinson’s, diabetes, cancer, depression and restricted mobility.

Developed by 15 leading health and social care charities, our aim is to help you find ways to be active that works for you and your health condition. We understand what works is unique for you and can change every day.

Dementia Friendly Hampshire

We are an independent charity, working to help people affected by dementia live a good life, with opportunities to participate in and contribute to their communities and be valued for who they are.

We help to set up and supporting local Dementia Action Groups , get involved in innovative and exciting Projects and Partnerships and signpost people to advice and information on dementia. Anyone who is keen to make a positive difference to the lives of those affected by dementia is very welcome to join us

Princess Royal Trust for Carers

A specialist care organisation, operating in Hampshire for over 30 years, now delivering the Countywide Carer Support Service for Hampshire County Council.

We provide a range of support and services for carers: emergency care plans; access to available allowances and grants; gaining support from your GP; a range of activities, events, exercises and tutorials to support you in your carer role. Carers and Patients Support Hub

A service provided by the Southern Health NHS Foundation Trust to provide support in helping to find the right NHS services or seeking advice about them. The service also offers a platform for feedback to the Trust.

Chichester Wellbeing

Chichester Wellbeing is a friendly and impartial service which comes from your local authority and other partners, our services are completely free to users. The Wellbeing service can help you to find local wellbeing information and services. Topics include: Quit Smoking, Weight Loss, Falls Prevention, HeartSmart Walks, Alcohol Support, Mental Health and many more.

Community First

Community First is a registered Charity operating in Hampshire which enables individuals and local groups to be the difference they want to see in their local communities.

Through connecting and engaging with the community and its needs we’re able to provide essential support services including: Transport and Mobility Services; Wheels to Work; Community and Sports Facilities; Volunteering; Home & Gardening Services; Young Carers support and Training and Counselling

Get up and go East Hants

A large range of activities in the East Hants area covering Rowlands Castle, Horndean and further north. Includes chair based yoga and pilates, walking football and many other activities for all ages and abilities. Click the link below to find out more

Citizens Advice

Citizens Advice Havant is a local charity that offers free confidential advice in person, with topics including welfare benefits, housing, employment and debt. Practical help is available with form filling and letter writing. Interpreters can be accessed if needed.
There are drop-in sessions on the 1st and 3rd Thursday of every month in Emsworth Baptist Church from 10am to 2pm. Our office in the Meridian Centre Havant is open Mon. to Fri. 10am to 3pm by appointment. Drop-in subject to availability.
